THE CANCER IS BACK

Luna had a follow up ultrasound and spleen and liver biopsy with Dr. Clemans last week. The cancer is back. The lymphoma is in her spleen and now her liver too. The chemotherapy was not working.

We had a decision to make, and it took us almost a full week to make it.

Do we keep going?

We looked at our goofy, happy, playful, sassy dog and decided YES, we will keep going.

Are we crazy? YES.

Are we right? I DUNNO.

But we are comfortable with our decision.

Next - five IV injections of a fairly new cancer fighting medication called Tanovea. One injection every three weeks. If it does not work or Luna feels like crap, we will be done. Done done. This is it. But this could give us several more months with Luna. And for us, that is the best news we could hope for.

In the words of the great Dr. Sue, Cancer Vet, ”Already a statistics buster. Kick butt Luna!”

And so she shall.

It is difficult to write about Luna right now because lately we have all been a little bit apprehensive.

But you have been with us through it all, and I don’t want you to miss this chapter just because it is a scary one.

Luna is still the dorky, goofy, fun dog she has always been. Right now she is demanding canned food from Russ. (Update: He caved.)

In December, Dr. Krapfl did an abdominal ultrasound on Luna and found irregularities in her spleen. He did a recheck ultrasound in January, and her spleen was even more abnormal. We did blood work, including a clotting profile (tests to make sure Luna is able to stop bleeding in a normal amount of time), to assure the next procedure would be as safe as possible.

Dr. Clemans did an aspirate (small sampling of cells) of her spleen.

The cancer is back.

We restarted chemotherapy last Wednesday. Her next dose is tomorrow.

Luna is also on Lasix.

“Why Lasix?” I asked Julie at VCA MidWest Vet. “I don’t know,” she said. “I will find out.”

The purpose of the Lasix is to prevent hemorrhagic cystitis - bleeding into the urinary bladder. It is new since we did chemotherapy last time, and Dr. Clemans has added it to her protocol.

I hung up with Julie and yelled at Russ, “Dr. Clemans is practicing cutting edge medicine! CUTTING EDGE!” After a few minutes, he said, “so…cutting edge, huh?” and went back to reading. He was, I am sure, quietly impressed.

Between Dr. Krapfl and Kelly and everyone at Gentle Doctor and Dr. Clemans and her team at VCA MidWest Vet, and all of you praying and cheering her on…this little dog has a great team surrounding her. Beautiful things are bound to continue happening.

Luna has finished her chemotherapy!

Tuesday was her last round of medication. All of her friends at VCA MidWest Vet signed a pink scarf for her with encouraging notes and love. She fell asleep with her pink scarf on, surrounded by love and support.

We are not done fighting. Luna has quarterly check-ups to look for the return of the cancer, and we will be watching for signs of its return at home as well. She may need to go through a second and even a third round of chemotherapy. We will cross those bridges at a different time. This time is for celebrating <3

We check Luna's complete blood count before every chemotherapy event to assure her white blood cells are not too low for her little body to handle chemo.

This week when we checked, her white blood cell count was way too HIGH. Dr. Clemens hospitalized her and went searching. Our biggest fear was that her cancer had spread.

It turns out Luna has a urinary tract infection, which is very treatable. She stayed overnight at the hospital last night and will come home tomorrow if all continues to go well. We went to see her last night.

Thank you for your prayers! The little goofball continues to hang in there.

Besides the stellar medical care from the doctors and team at VCA Midwest Veterinary Referral and Emergency Center, they have gone above and beyond taking care of Luna and taking care of us. The team let us visit last night and is letting us visit again tonight. They have sent us pictures and texts with updates that have meant the world to all four of us. It is so unnerving to not have Luna here - she is very much a snuggly and in your face dog, and her presence is sorely missed! The sweet updates have taken the edge off for all of us and made this separation bearable.

Our little dog Luna is doing so well you guys!

She is over halfway through with her chemotherapy for lymphoma.

I JUST learned from Dr. Clemans, her internal medicine specialist doctor who is directing her care, that Luna gives them all kisses! I am so happy she is such a good sport there and is sweet to them. She IS sweet, but she is onery too, so I was not sure which Luna she was at VCA MidWest Vet. Apparently, Good Luna. Yay.

LUNA LOVEGOOD - CHAPTER 1 - THE BEGINNING

LUNA LOVEGOOD - CHAPTER 1 - THE BEGINNING

Luna was rescued by the Nebraska Humane Society from a hoarding situation. She soon became very ill. She had sky-high liver values and bile acid values (a measure of liver function).

She came to the hospital where I am a veterinarian to be assessed and have an ultrasound done. When liver values are so far out of balance in a young, small breed puppy, what we suspect is a liver shunt.

A shunt was not found, and it was determined that she had a condition that could not be surgically fixed. The Nebraska Humane Society team, in an effort to spare the puppy suffering, decided to have her euthanized.

The day of the puppy’s ultrasound was my day off. I received medical updates and the final decision to have the puppy euthanized in a series of several emails, all of which came at once in the late afternoon.

I sat on the bed and bawled. My husband Russ came to check on me, and I yelled, “I take one day off, and puppies DIE!”

Suspecting this was not an accurate statement, Russ gently took my phone out of my hand and read the emails. He said, “Couldn’t we take her?” Still in complete despair mode, I said “She’s probably DEAD!” Of course she was not dead. Dr. Thomassen said she would not euthanize the puppy unless she knew it was absolutely a decision agreed upon by everyone.

So we accidentally adopted a puppy!

The other doctors thought I was crazy to adopt a broken puppy, but I think they understood. We are vets. It is what we do.

I talked with my daughters. Adopting a puppy with an unspecified liver disorder is one of the best ways that I can imagine to ensure heartbreak. I told them we were adopting a puppy we knew was sick and that her lifespan may be shorter than other dogs. There was no doubt in their minds that this puppy was meant to be a Finch.

On the way home with the puppy in my lap, I said to Russ, “She’s blond. She’s really sweet. She’s pretty spacy.” (To her credit, she was still recovering from a very severe liver issue, and has since proved much sharper.) When we got home, I said to our daughters, “Let’s name her Luna Lovegood.” They cheered, and we had our name.

Our next step was a simple liver test. AND…the blood work came back completely normal!! And now I get to ponder what in the world happened.

  1. Knowing Luna now, it is probable that she ingested something she should not have, causing her extreme illness and liver issues.
  2. Knowing the very first place she lived had so many dogs, it is probable she ingested some urine at some point in her young life.

So my top guesses are a liver toxin from which she has recovered or leptospirosis from which she has recovered. While I know of course, that she will one day break my heart, I am so glad it is not on the second day or the second month that I have known her. She has been a challenging, exhausting, strange, wonderful, goofy amazing puppy. I am so happy she is a Finch.

I dunno how many of these weekly no news updates you would enjoy. Personally I would be happy with about 500! Luna had another set of IV chemotherapy and is doing great! She jumped on Russ's head when she was discharged, entertaining the whole VCA MidWest Vet team. I'm glad they get to see her as her normal, dorky self!

On the subject of Luna's normal dorky self...Jen Rios gave Joy the Puppy a cardboard roll (Thank you Jen!!) Joy has been playing with it non-stop. Luna found a torn off bit of cardboard from Joy's roll and has been playing with IT non-stop.

So things are pretty normal around here! Everyone is happy and well :)

Luna has now been through four sets of chemotherapy, each about a week apart. The first two were IV treatments at VCA MidWest Vet, the third was at oral medication at home and the fourth was IV treatment again.

Tomorrow will be Luna's next IV treatment, pending good news from her complete blood count and urinalysis today. (Thank you Kelly! For everything.)

Luna does not mind her half day stays with me on Mondays, and she loves her visits with Dr. Clemans and Brandy and the team on Tuesdays. We have gotten into a routine, and none of it seems to be scary to her.

She had a rough couple days last week where she would not eat, but is now eating chicken and rice well. We may have been had in typical Chihuahua fashion, but she really did seem nauseous, and now is all better. We will either wean her back onto dog food or transform her chicken and rice into a balanced diet using the website my friend Dr. Kanne showed me, Balanceit.com.

So far, so good you guys! Luna is feeling great and acting onery, just what we hoped! It has been warm enough for evening walks again, so Russ and I have been walking Joy and Luna, sometimes around the block, and sometimes to the elementary school to play. We have all been loving that time, and the whole family has loved our time with Luna.

I will stay on top of getting updates to you! Hopefully they will all be this normal!

Luna is able to jump onto furniture again. She no longer collapses in the hind end on walks.

Today Kelly called for Luna’s flow cytometry results. Flow cytometry is a way to further differentiate cells, in this case, Luna’s blood cells.

Luna has stage 5 (the worst - involving the bone marrow) cancer, B-cell (better than T-cell) lymphoma. 

I do not know what to make of this. I strongly suspect this is more bad news than good. Someone much smarter than me (that is, internal medicine specialist Dr. Clemans) will be have to interpret these results.

It will not change treatment, but more information is usually better than less.

Luna’s disease is a complete mind <game>. 

She has a rapidly progressing fatal disease and yet feels great. She looks like herself, yet cancer cells are taking over her body and shutting it down. It is such a strange situation to be in - celebrating her “health” while mourning her upcoming demise. I don’t like it, but I like it better than if she already felt sick. I don’t know what to compare it to, except a very accelerated mourning of a pet’s mortality that you always know will eventually catch up with them. It’s like going from having a three year old dog to having a fifteen year old dog, without getting the years in between.

You just keep being healthy there Joy. I know your lymph nodes are normal because I've felt them 87 times since Luna's diagnosis.

Tuesday was Luna’s first chemotherapy appointment. She spent the day with Dr. Cooley and Brandy and the rest of the MidWest Vet team.

Luna’s chemotherapy medicine was prepared in the morning. An IV catheter was placed in her front leg. She received an IV drip of her medication in the early afternoon. Then she was monitored for signs of illness for the rest of the afternoon. All went well, and Luna did great.

Russ worried that Luna would be wiped out after her first Big Day. He waited anxiously in the waiting room. Luna saw Russ and ran straight to him and jumped up onto the chair with him. She was so happy to see him, and he was so happy she was still her goofy, high energy self.

That night, Luna was still fine, but the rest of us were wiped out. While we usually go to Finches’ for dinner Tuesday nights, we stayed home with Luna and rested.

We bought our nephew Arthur an elephant chair for Christmas. Luna promptly decided it was hers. So we bought Luna an identical one. Now she and Arthur can be elephant buddies.

With Dr. Clemans' blessing, we started prednisone early. It is part of the chemotherapy protocol, but started too soon can interfere with the rest of chemotherapy's effectiveness.

Luna was becoming pretty mopey and was unable to jump onto the bed or the couch. Now she can do both again.

I have to admit, this picture was taken a good half year before Luna's cancer diagnosis.

We met with Brandy the veterinary technician and Dr. Clemans, one of the internal medicine specialists at VCA MidWest Vet. They are both so nice. We were completely tidal waved with information. Even knowing all the big words, I had a difficult time keeping up with how they went together and what they meant for Luna.

Luna has at least stage four (of five) lymphoma, and possibly stage five. The younger the dog, the more aggressive the lymphoma tends to be. Although I read in the Five Minute Veterinary Consult that lymphoma is "rarely cured," it turns out the authors were just trying to say gently that it is never cured. This is a fatal disease. Always. I knew that. I had to be told that by an expert.

The CHOP chemotherapy protocol for lymphoma is twenty-five weeks long.

Dr. Clemans is not certain Luna will live through the twenty five weeks.

She handed me a box of Kleenex and continued. I tried in vain to pull it together. I have not been okay since hearing this.

Luna will have a complete blood count (blood work) done at Gentle Doctor every Monday. We will purchase the IV chemotherapy medications at Kohll's pharmacy once a week (no veterinarian discount!) and drop Luna off at VCA MidWest Vet every Tuesday. There she will be evaluated by Dr. Clemans. Brandy and the rest of the team will administer chemotherapy. She will come home in the afternoon. Luna loves Brandy and Dr. Clemans, so this once a week day long stay should not be a problem.

Chemotherapy for dogs tends to be much gentler than chemotherapy for humans. Luna will not lose her hair, and will probably not develop gastrointestinal signs or even be tired or mopey.

I had a heads up from two kind client families about the costs associated with chemotherapy for lymphoma. Brandy went over an itemized estimate with us. The price is steep but the potential  benefits are priceless and worth the investment for us. Many clients have chosen not to go through chemotherapy with their pets, which was the absolute right decision for them too. Our goal is to allow Luna as much high quality time as we possibly can. We will quit when she is no longer loving life. I have not wrapped my head around that yet though.
